BSA Medical Forms help prepare the troop leaders to handle a medical emergency whether at a weekend camping trip, troop meeting or high adventure. Our troop's Medication Form provides troop leaders with instructions for administering medications to a Scout at activities.
There are three different types (classes) of medical forms which give different levels of detail and have differing expiration dates.
Class 1 Medical Form
... for Meetings and Weekend Activities (less than 72 hours)
The Class 1 medical form has the basic information we need to give first-aid to most healthy scouts and scouters on a week-end camping trips or at troop meetings. The class 1 medical form is for trips less than 72 hours for healthy individuals. It does not require a doctor's signature and is filled by a scout's parent or guardian. We also want these forms on file for adults. The class 1 medical form lists who we should contact in the event of an emergency, what medical conditions and/or allergies they have and when they were vaccinated. It also gives us permission to start/seek treatment and lists the person's health insurance.
A class 1 form is reviewed yearly to make sure there have been no changes to the person's health, insurance or contacts.

Class 1 & 2 Medical Form
... For Summer Camp, or
... For Any Activity Longer Than 72 Hours
You'll never see a class 2 medical form without a class 1. The class 2 form is filled out by a physician and gives additional information you'd find on the scout's school physical form. Class 2 is for outings over 72 hours such as summer camp for those that are healthy and are under 40 years old.
Together the class 1 and 2 forms give even more information about how the scout is growing and developing and helps the medical staff at the camp should they need to treat the person.
The class 2 portion of the form expires every 3 years.
The class 1 portion is reviewed yearly.

Class 3 Medical Form

A class 3 medical form combines the information in a class 1 and 2 form with additonal and more detailed information on a person's health and their ability to participate in the activities in scouting. Several sections are filled out by the adult or by the scout's parent/guardian and severel other sections are filled out by the physician.
A class 3 medical form is needed for all high adventure trips (and there are special forms for Philmont, Sea Base and Jamboree), and adults who are 40 and over going to summer camp or other outing over 72 hours. A class 3 is also used for anyone with a "significant" health condition. Significant health condition includes someone who is seeing a doctor twice a year or more about a medical condition such as diabetes, severe asthma, heart condition to name just a few.
Class 3 medical forms are filled out yearly.

Troop Medication Form
... For Scouts Needing Medications During Activities
The troop medication form helps the tour leaders know what medication a scout needs to or may need to take and when it needs to be or can be taken. The medication may be one the scout takes routinely for ashtma, allergies, ADD/ADHD, heart condition, etc. or it may be one he takes if needed such as rescue inhalers, allergy meds needed sometimes but not all the time, antiacids, etc.
This form is completed for each trip. All medicines should be in original containers, containers should be placed in a zip lock bag along with this completed form, and checked in with the Tour Leader at the time of departure. The Tour Leader holds and dispenses all medications, returning them at the end of the trip.
