BSA Medical Forms help prepare the troop leaders to handle a medical emergency whether at a weekend camping trip, troop meeting or high adventure.  Our troop's Medication Form provides troop leaders with instructions for administering medications to a Scout at activities.

There are three different types (classes) of medical forms which give different levels of detail and have differing expiration dates.

for Meetings and Weekend Activities

This form has basic information we need to give first-aid to most healthy scouts and adults on a weekend camping trip or at troop meetings.   It is for trips less than 72 hours; it should be used for healthy individuals.  It does not require a doctor's signature.  For Scouts the form is filled out by a scout's parent or guardian. 
   The Class 1 form must be updated annually.

... For Any Activity Longer Than 72 Hours

The Class 2 form must be completed by a physician and gives additional information beyond the Class 1 form.  Class 2 is for outings over 72 hours such as summer camp.  The Class 2 is for all Scouts.  It can only be used by adults if they are healthy and are under 40 years old.
      The Class 2 form expires after 36 months.

Class 3 Medical Form

... For People Over Age-40 on trips of 72+ hours, or
... For People With "Significant" Health Condition

A Class 3 medical form combines the information in the Class 1 and Class 2 forms with additonal and more detailed information on a person's health and their ability to participate in Scouting activities.
     Some of the Class 2 information is filled in by the the adult or by the Scout's parent/guardian, and other sections of the Class 3 are filled in and signed by the physician.
     A Class 3 medical form is required for troop high adventure trips (note: there are special forms for national high adventure camps and jamborees).   The Class 3 is for all adults who are age 40+ who attend any event more than 72 hours.  
     A Class 3 is also used for anyone (Scout or adult) with a significant health condition.   "Significant" health conditions include those which require a doctor visit 2+ times annually and include conditions such as diabetes, severe asthma, a heart condition, etc. 
       Class 3 forms expire after 12 months.

... For Scouts Needing Medications During Activities

Our troop medication form helps the adult tour leaders know what medications a Scout requires during a trip or meeting.  The medication may be one which the Scout takes routinely (e.g., for asthma, allergies, ADD/ADHD) or it may be one he takes only if needed (e.g., rescue inhalers, antacids).

This medication form is completed for each trip. All medicines should be in their original containers and held in a zip-lock bag along with the completed medication form.  Check in this bag with the Tour Leader at the time of departure.   The Tour Leader will hold and dispense all medications, returning them to the Scout/parent at the end of the trip.
